Health & Wellness Benefits
Medical Coverage
Employees in Kansas, Colorado and Montana
SCLHS offers four medical plan options administered by Blue Cross Blue Shield of South Carolina. The Base PPO Plan and Plus PPO Plan provide benefits both in-and out-of-network. Annual deductibles, co-payment levels and out-of-pocket maximums vary based on where services are received and the plan you select. The Supplemental Plan is designed to "tie in" with another plan the participant may have. This plan pays the first $500 in covered expenses and then pays 20% of remaining expenses.
The Consumer Driven Health Plan (CDHP) encourages employees to act as consumers and to make informed decisions with regards to their family's health care needs. The CDHP gives the employee the ability to take control of his or her health care dollars and determine when and where those dollars will be spent.
Employees in California
Employees working in California have access two three medical plans. Two of the medical plans are offered by Anthem Blue Cross of California. One Plan is an HMO plan that provides in-network only benefits. The second plan is a PPO plan and does offer both in and out-of-network benefits. The third plan is an HMO plan that is offered by Kaiser.

Dental Coverage
The SCLHS offers two dental care options. The Plus Plan pays 100% of preventive and diagnostic services. Lifetime maximum of $2,000 for orthodontics and TMJ. The Base Plan pays 100% of preventive and diagnostic services. Lifetime maximum of $1,000 for orthodontics and TMJ.

Vision Plan
This benefit covers annual vision exam plus eyeglass and/or contact lenses every year and new frames every two years for each covered participant. The plan pays 80% of eligible costs up to maximum of $250/person.

Preventive Care
The SCLHS self-insured plans offer Wellness Benefits that include coverage for mammograms, well woman care, prostate screenings, colonoscopies, bone density screenings, well-baby care, immunizations and flu shots.
Additionally, the Wellness Benefit offers each covered member $300 per year to be used for specific wellness services.

Personal Health Assessment
An online Personal Health Assessment is available for employees to complete. This online tool gathers information related to nutrition, tobacco and alcohol use, vehicle safety, physical activity, stress, depression, current health, health history and biometrics and creates a printable individual report that you can share with your health care provider. All personal information on the Assessment is confidential and is not shared with SCLHS.

Family and Medical Leave Act
The Family and MEdical Leave Act provides eligible employees an opportunity to take up to 12 weeks of unpaid, job-protected time off of work for certain reasons. This program is administered through your local Human Resources department. The plan provides three different options. The options include:$10,000 spouse/$5,000 child;$20,000 spouse/$10,000 child; or$50,000 spouse/$25,000 child. The amount of dependent life insurance cannot be more than your life insurance coverage.

Employee Life Insurance
FULL TIME: The plan provides the opportunity to purchase up to five times your annual base salary in life insurance, up to maximum of $1,000,000.
PART TIME: The plan provides $15,000, $25,000 or $50,000 coverage.

Basic Life Insurance is provided to full-time employees equal to one times your annual base salary, with an option to purchase an additional four times annual base salary up to maximum of $1,000,000. Part-time employees receive a basic life insurance benefit in the amount of $15,000 with an option to buy up to $25,000 or $50,000 coverage.

AD&D Insurance
FULL TIME: The plan provides the opportunity to purchase up to five times your annual base salary in AD&D coverage, up to a maximum of $1,000,000.
PART TIME: The plan provides $15,000, $25,000 or $50,000 coverage.

Basic AD&D coverage is provided to full-time employees equal to one times your annual base salary, with an option to purchase an additional four times annual base salary up to maximum of $1,000,000. Part-time employees receive a basic AD&D benefit in the amount of $15,000 with an option to buy up to $25,000 or $50,000 coverage.

Long Term Disability
SCLHS offers three LTD options that provide salary continuation of 40%, 50% or 60% of monthly salary up to a maximum benefit of $6,000, $7,500 and $9,000 respectively. Benefits are integrated with Social Security, and payments begin following 90 days of disability and may continue until age 65.

Retirement Plans
Defined Contribution Plan
The SCLHS Defined Contribution Plan provides a 5% basic employer contribution toward the participants retirement needs and to encourage individual savings for retirement. SCLHS provides a 50% matching contribution of first 6% a participant contributes to the Retirement Savings Plan. The vesting schedule is as follows: 3 years - 50%, 4 years - 75%; 5 years - 100%.
